New Toolkit on Care Transitions for Persons with Mental Health & Co-Occurring Conditions

The Centers for Medicare and Medicaid Services with guidance from the Substance Abuse and Mental Health Services Administration published the Care Transition Toolkit for Persons with Mental Health & Co-Occurring Conditions. The toolkit is designed to help people with mental health conditions to seamlessly navigate their way from one care setting to another. . The toolkit can help keep track of personal health information, be part of the individual’s care planning process and share medical preferences with the provider support team.

NASHP Conference Materials Now Available

The National Academy for State Health Policy (NASHP) hosted its 28th Annual Conference last month. Materials and presentations are now available from their preconference and conference sessions.

Medicaid ACOs State Profiles

The Center for Health Care Strategies (CHCS) published a brief providing state profiles on Medicaid Accountable Care Organizations (ACOs) in nine states. For each state, the profile outlines key ACO infrastructure; details unique payment features, quality measurement approach, and data strategy approaches; and spotlights one of the state’s Medicaid ACOs. This set of profiles can help inform Medicaid ACO development in other states.
